CAREMARK (City North)
CAREMARK (City North), with offices at Broombridge Business Centre in Dublin 7, is owned and operated by Tom Duke. The office provides a home care service on behalf of the HSE and private Clients on the north side of Dublin.
"Home First" is a pilot initiative managed by a multidisciplinary team at Beaumont Hospital. The intiiative allows older people clinically discharged from hospital to be supported in their own homes.
CAREMARK (City North) are proud to be one of the care providers selected to work on this project. Prior to their discharge, a full assessment of the Client's needs is carried out by a multi-disciplinary team that includes Nurses, Physiotherapists and Occupational Therapists. A Care Plan is then decided by the Care Organiser, Public Health Nurse and Care Provider, with the client and their family.
Care provision includes assisting and supporting each client with all daily living activities, including personal care, domestic tasks and social and leisure activities.
Clients receive a number of visits each day depending on their specific needs. The multi-disciplinary team continuously monitor and review the service provided, with reviews completed on average every fortnight.

Amanda, Care Coordinator for the CAREMARK (City North) office holds a diploma in Social Care. Amanda's main responsibility is to ensure all care visits are allocated, to recruit suitable care staff and to maintain appropriate office procedures.
